What is Case Management?

York Support Services Network (YSSN) is a non‐profit, community‐based agency that provides case management services to children and adults who have developmental disabilities living in York Region.

Case management workers help individuals and their families to plan for the services needed to support the individual with a disability as well as connect them with services available in the community. The individual or family may wish to be connected to services such as:

Resource Days

YSSN sets aside dates for families who are on the waitlist and provides them with an opportunity to meet with a case manager face-to-face to answer questions, provide information, help with forms, etc. If you would like to meet with someone face-to-face, please call 1 866 257 9776 x 2574 for an appointment.  Dates for resource days are available in the YSSN Bulletin on our website at www.yssn.ca.
