Community Based

York Support Services Network is a community-based agency that offers a range of case management and community crisis services throughout York Region, to support individuals with a developmental disability and/or a serious mental illness. YSSN also provides services within the Children's Services Sector, offering Children's Case Coordination.

Well Established

Since our beginning in 1982, YSSN has a long history of supporting individuals and their families as they deal with the challenges of a developmental disability or serious mental illness. Guided by our Mission -- Fostering growth, independence and choice for people with developmental disabilities or serious mental illness -- YSSN is proud of the role we play as a bridge for our clients to the community, assisting them to receive the support they need to manage their disability or illness.

Aware

At YSSN, we understand the many issues our clients face -- lengthy waiting lists for services, lack of residential placements and community supports, confusion around what services are available, and challenges in accessing the help that they need. We also are aware of the changing and diverse needs of our region's communities and are committed to meeting those needs.

Well Connected

YSSN is well connected within health and social services. We participate in a variety of activities that aim to:

Accountable

At YSSN, we are committed to remaining transparent and accountable to the communities and individuals we support. As a service provider within the Central Local Health Integration Network (CLHIN), YSSN was required to negotiate a Multi-Sector Accountability Agreement (M-SAA) with the CLHIN for two years ending March 31, 2011. This agreement details a number of financial and performance obligations that YSSN has pledged to meet. A copy of the YSSN-CLHIN Accountability Agreement is accessible below:

YSSN is a non-profit organization that receives funding from the Ministry of Community and Social Services, the Ministry of Children and Youth Services, the Central Local Health Integration Network, and the Ontario Trillium Foundation. There is no fee for our services.
