Developmental Services Program

Case Management

Persons using the Case Management Program have a significant delay which occurred before age 18 and serious limits in day-to-day functioning. Common diagnoses include: Downs Syndrome, Fragile 'X', or Pervasive Developmental Delay.

Persons using our Case Management Services are supported by a Case Manager, who works with them to coordinate formal and informal services to meet their unique needs.

Service System Response Unit

Partnering with other service providers in the Developmental Services Sector, YSSN hosts a variety of programs for adults and children who have developmental disabilities and their families.

Coordinated Response to Community Participation (Day Supports) and Accommodation Services
YSSN's Service System Response Unit (SSRU), works with community agencies to coordinate access to a range of accommodation, day supports and respite services.

Systems Planning
YSSN collects information on needs in York Region to support the ongoing development of the service system.

Case Resolution for Complex Situations
A case resolution process has been developed to effectively plan for adults whose needs require the support of more than one agency. Using a person-centered approach, Access Workers develop cross-agency, cross-sector service plans to meet the care needs of each person.

Children's Case Coordination

Children's Case Coordination, hosted by YSSN, addresses the needs of children, youth, and families who have complex social, emotional, developmental and/or physical needs. Using a strength-based planning approach, Children's Case Coordination works with agencies to develop coordinated responses and identify solutions where service gaps exist.

YSSN hosts two services that support families who use respite services:

CHAP Program (Community Helpers for Active Participation)
CHAP Program maintains a respite worker registry to assist families in hiring staff. Resource information is available on the York Region www.respiteservices.com website.

Respite Access Service
A facilitator works directly with adults and their families to identify respite needs, develop individualized plans and link them to appropriate resources. Information is collected to assist in the planning and creation of new respite options.

Inclusive Recreation Resource Service

Working in partnership with the recreation sector, the Inclusive Recreation Resource Service focuses on promoting and facilitating the inclusion of all children and adults with a disability for participation in leisure activities of their choosing.
